At the suggestion of the regulator, performed Root Cause Analysis of a series of nine related incidents in a nuclear power facility.  Identified the common causes.  Presented results to the client and the regulator.  Client implemented corrective actions at a cost of $400k.  Regulator approved of changes.

At the request of the client, the regulator and the IAEA, performed a Root Cause Analysis of the catastrophic failure of an 800mm pipe for cooling water at a nuclear plant.  Identified the root causes and provided recommendations to the client.  Presented results to the regulator.  Regulator reviewed and accepted the report.  Changes implemented at the plant – $3M. Changes made to the regulatory process in country.  Changes made to the inspection processes through worldwide announcement.

Root Cause Analysis of dry cask storage failures for the client.  Provided report and presentation for the client and the regulator.  Developed recommended corrective actions for the client.  Client implemented the corrective actions and resolved the problems.  Cost of repair – $1.5M (est.)  Damage settlement not disclosed.
