Led team and performed a Root Cause Analysis into failures of a plant to remain within the licensing basis for operations for 15 years.  Prepared report for bringing the plant into compliance.  Plant committed to making the necessary changes.  In part related to the findings and related costs, the regulator requested that the plant permanently shut down at the end of the fuel cycle.  Cost of shut down – $1B (estimated).
