ABOUT
Dorian is an internationally recognized expert in Incident Investigation, Root Cause Analysis, Safety Behavior and Human Performance. For more than thirty years in the industry, he has pioneered projects or training initiatives for clients in: Africa, Antarctica, Asia, Australia, South America, Europe, and North America. He has led major investigations at commercial and government facilities that include: Nuclear, Manufacturing, Chemical, and Petroleum. And, he has conducted training programs for more than 200 client organizations, and collectively has instructed more than 10,000 students.
Dorian has managed more than 100 major Incident Investigations for client organizations that include: Amerada Hess, Apache Energy, CenterPoint Energy, Chevron, First Energy Corporation, International Atomic Energy Agency, Southern Nuclear Corporation, TAPS/Alyeska, TVA, the US Navy, USNRC, and others. He served on the North Slope Spills Expert Panel and developed recommendations for reducing oil spills on the North Slope of Alaska. His efforts have led to national and international changes in the conduct of business in the nuclear, oil, and gas industries.
As a developer/instructor, Dorian led the first Institute of Nuclear Power Operations (INPO) training program on Incident Investigation and Root Cause Analysis, and participated in the initial development of the Human Performance Enhancement System (HPES). He also developed the Model Root Cause Analysis and Corrective Action Program that has become the global standard for the energy industries.
Dorian’s current emphasis includes: Incident Investigation/Root Cause Analysis, Corrective Action Program Development and Assessment, Safety Culture Assessment and Enhancement, Regulatory Recovery Efforts for Nuclear Power, Electric Transmission, and Gas Transmission and Distribution..

Maria J. Esteban
Maria Esteban is fully independent from any vendor, utility or governmental organizations. She has more than 35 years of experience in nuclear and fossil plants, as a consultant with architect and engineering companies. She has broad experience in assessing Operating Experience, Root Cause Analysis, Performance Improvement, with Spanish and international projects.

Dr. Ivan Vrbanic
Dr. Ivan Vrbanic is a principal in APoSS, a Croatian company. He is a senior consultant in safety analysis (quantitative and qualitative). Dr. Vrbanic’s most recent projects include: Technical Leader for the project of Angra 1 (Brazil) Level 2 PSA, Low Power and Shutdown PSA, External Events PSA and Risk Monitor (sub-contract to ENCO) (2018); consultation to KINS, South Korea; in the field of risk significance assessment (sub-contract to Conger) (2017); participation in On-Site Assistance to Armenian NPP / Metsamor (sub-contract to ENCO) (2016); EC Directorate – General Research & Innovation under the framework of H2020 (2017); NEK Containment Event Tree update / upgrade (2016-2018); and consulting Electrabel, Belgium, in the field of Internal Fire PSA (2016-2017) (sub-contract to SiTe).
