Performed Root Cause Analysis of a loss of service to 15,000 customers in the Midwest in mid-winter due to a water intrusion into a gas pipeline.  Identified causes of the intrusion.  Recommended changes to the energy management system and SCADA system.  Client implemented changes.  No further problems with frozen valves.  Legal settlement not disclosed.

Root Cause Analysis of failures in the weld inspection quality assurance process at multiple pump stations on an 800-mile pipeline.  Identified the root causes.  Provided the client with recommended corrective actions.  Client implementing system wide changes to the weld inspection and quality assurance program.  Cost of incident – $2M in lost production and cost of repair.
