Root Cause Analysis of oil spill in the Gulf of Mexico resulting in environmental damage to wetlands and the deaths of hundreds of brown pelicans (a protected species) at a cost of $7M. Identified root causes. Provided client with recommended corrective actions. Corrective actions implemented on the subject platform and all other platforms owned by the client in the Gulf. Cost of implementation – $300K

Led the Root Cause Analysis of the loss of a $500M offshore oil platform.  Evaluated regulations, design standards, design, fabrication, installation, etc. of the platform.  Identified the root causes of the inverting of the platform.  Recommended changes for future design and modifications to other similar structures currently in use.  Client implemented modifications on the $1B structure under construction and modifications to other similar platforms.  Regulator recommended industry-wide inspections of similar structures.

Led the Root Cause Analysis of the fatalities resulting from a pipeline explosion due to over pressure.  Identified root causes.  Provided client with recommended corrective actions.  Regulator required inspections of all similar devices in the Gulf of Mexico and recommended replacement of error provocative designs.  Total cost – $2-5M estimated.
