Led the Root Cause Analysis of the ground safety review process for payload safety on the space shuttle.  Identified weaknesses in the ground safety program.  Changes made by NASA to reduce the risk of incompatible payloads.  No incidents in the history of the program related to incompatible payload interactions.

Performed a Root Cause Analysis of the solid rocket motor design, manufacturing and testing process.  Identified root causes of potential problems with the rocket motor development and testing process.  Recommended corrective actions to the client who presented them to the government agency purchasing the motors.  Client implemented the corrective actions.  Cost of implementation estimated at $10M.
